What are chronic diseases?
Chronic diseases are long-lasting health conditions that typically develop gradually and persist over time, often requiring ongoing management rather than a one-time treatment.
Chronic diseases can last for months, years, or even a lifetime, and they may affect a person’s daily functioning and quality of life. Common examples include diabetes, emphysema, asthma, osteoporosis, arthritis and heart failure.
While many chronic diseases cannot be cured, they can often be controlled through medication, lifestyle changes, and regular medical care.


Benefits of seeing your GP
A Chronic Disease Management (CDM) plan is available to patients with a chronic or terminal medical condition that has been, or is likely to be, present for six months or longer.
The CDM plan covers up to 5 allied health vouchers per calendar year.
Eligibility is based on a GP’s clinical judgment that a structured approach is needed.
